开元真人 Integrated School Application Requirements
for AY 2025-2026 (Nursery to Grade 10)

ELIGIBILITY

A. Age Requirement

  • Nursery applicants must be at least 4 years old by September 14, 2025*.
  • Kinder applicants must be at least 5 years old by September 14, 2025*.

Note: Cut-off date of Nursery and Kinder age requirement is dependent on school openingdate.

B.Grade Requirement

  • Above average percentile rank in the admissions test
  • A General Average of at least 85 and above or its letter grade equivalent
  • No grade below 80 in any subject
  • No grade below 80 or its equivalent in Conduct / Deportment

APPLICATION PROCEDURE

STEP 1: Accomplishment of requirements

  • 1. Application Form
  • 2. 2×2 size most recent photo (white background)
  • 3. Current Report Card (available grades of AY2024-2025)
  • 4. Report Cards for the past two (2) School Years (AY2022-2023 and AY2023-2024)
  • 5.听ECCD Checklist (for incoming Grade 1 students only)
  • 6. Photocopy PSA/NSO Birth Certificate
  • 7. Guidance Transition Certificate (accomplished by the Guidance Counselor or class adviser/ subject teacher)
  • 8. Two (2) Recommendation Forms (accomplished by the Principal/Class Adviser/Subject Teachers or other school personnel)
  • 9. Proof of Payment (Application Fee)

*Kindly share the Google form link provided below with the authorized representative of your聽child鈥檚 current school. They can use it to submit/upload the completed Recommendation Forms聽and Guidance Transition Certificate (items #7 and 8). The required 开元真人 forms can also be聽downloaded through the provided link:

Additional Requirements for:

  • Filipinos with dual citizenship/Born in the Philippines or abroad – Valid Passport, Documentation for Acquisition/Reacquisition/Retention of Filipino Citizenship
  • Non-Filipinos/Born in the Philippines or abroad – Valid Passport
  • Students with other visa such as 47a2, 9G etc. – Photocopy of Passport bio-page and stamp of latest Philippine arrival, scanned copy of student and parent/s I-Card (front and back), SRRV, SIRV and Working Visa card

STEP 2: Submission of Requirements

  • Scan the application requirements from items #1 to #6 and email to [email protected] with below subject heading for easy reference.
  • SUBJECT: Applicant Name_Level Applied for (Example: Juan Dela Cruz_Application for Grade 7)

STEP 3:聽Verification of Documents聽

  • Await email acknowledgement from the ORBE (formerly OASIS) Admissions.
  • If requirements are deemed complete, a payment reference number will be sent via email within 2 to 3 working days.

STEP 4: Payment

  • Pay the non-refundable/non-transferable Application Processing Fee of PhP800.00 and send payment proof via email. Fee for applicants from Philippine public schools is
    waived.

 

STEP 5: Application Update

  • Application update and test permit to be released within 5 to 7 working days upon receipt of complete application requirements and admission fee payment proof.

Important Reminders

  1. Ensure completeness of requirements. Incomplete applications will not be processed.
  2. Applicants with non-numerical (pass/fail) grades on Report Cards in all terms/semesters/quarters are required to submit the following through email:
    • Certification signed by the School Registrar that no numerical grades were given to students.
    • Legend of grading equivalency.
  3. Any false information and/or fraudulent document, either submitted through email or in physical form, will automatically nullify the application and permanently disbar the applicant
    from pursuing any level of study at De La Salle University.
