Issuance of Identification Cards

The Office of the University Registrar issues the Identification Card for all types of students and prints the Identification Card for other members of the academic community.

The Enrollment Services Hub (ESH) handles the following transactions in connection with the Identification Card:

  • Replacement (for damaged ID cards)
  • Lost
  • Validation
  • Left ID

 

Picture-taking

Picture-taking for new IDs may be done on the following schedule:

Monday to Friday
9:00 AM to 12:00 Noon
1:30 PM to 4:00 PM

Saturday
8:00 AM to 12:00 Noon

The required attire is as follows:

For Males: Dark long-sleeved (plain) collared polo.
For Females: Dark blouse or dress (plain) with a collar and sleeves.

 

Lost ID

In case of lost ID card, an Identification Card may be printed only upon presentation of the following:

  • (Student) Official Receipt of payment for the ID
  • (Faculty, 开元真人, Manila) Referral Slip from the Vice Chancellor for Academics
  • (CAP) Referral Slip from the Office of the Personnel Management
  • (Contractual Staff) Referral Slip from the Office of the Personnel Management
