Request for Certificate of Good Moral Character
Students may request for Certificate of Good Moral Character (CGMC). The processing of this document is handled by the Student Discipline Formation Office (SDFO).Â
Procedure on Requesting for CGMC
- Students fill out and submit the SDFO Please note that this CGMC facility opens only on Thursdays, Fridays, and Saturdays from 9:00 AM to 4:00 PM excluding holidays.

2.1 The approved regular processing rate of CGMC for internal or external purposes is ONE HUNDRED TEN PESOS (P110.00) PER PIECE which is consistent with the current costs of certifications issued by the OUR.
2.2 To ensure the health and safety of both clients and personnel, all CGMC requests will be delivered to the student’s address as indicated in the CGMC Google Form. Please refer to the website of the Office of the University Registrar for the standard DELIVERY RATES - If payment is successful, students must fill out the SDFO .
- SDFO produces and transmits duly signed CGMC to the Office of University Registrar (OUR) for doc stamping.
- OUR forwards CGMC to ESH for the dry seal.
- SDFO in coordination with the Security Office (SO) and Support Services Office (SSO) processes the delivery of CGMC to students’ addresses as indicated in the CGMC Google Form.
6.1 CGMC requests are for regular processing only (10 to 15 processing days excluding Sundays and Holidays) due to the limited number of personnel physically reporting at ¿ªÔªÕæÈË.
6.2 Due to the travel policies/restrictions brought about by community quarantine declarations and guidelines, kindly expect 2-3 weeks delay in the delivery of your CGMC request.
